Thomas - In the 'Table of Contents, Index or Bibliography' dialogue window, in the 'Type' tab, under 'Create From', is the 'Additional styles' box unchecked?

In a few instances I SOMEHOW managed to get almost what I wanted. Now, working on a different file, completed the ePub version, try to set up a TOC. Heading 1 + Heading 2 properly set, as far as I can tell. Tools -> Heading numbering set properly (I hope). Still, inserting a TOC that is supposed to show Heading 1 + 2 includes EVERY paragraph of the entire document. Working for hour to set this straight, I still "do not get it". Could someone please point me to the thing that needs fixed.
